What this policy covers
This policy describes how Sober Lyfe Treatment (“we”, “us”) handles information collected through this website, sl-treatment.com, and through calls and texts you place to the number shown on it.
It does not cover your health or treatment records. If you become a client, that information is governed by federal and Florida law, including HIPAA and the federal confidentiality rules for substance use disorder records (42 CFR Part 2), and by our Notice of Privacy Practices. Where the two documents overlap, the Notice of Privacy Practices controls.
What we collect
Information you give us
When you submit the “Request a Call” form we receive your first name, phone number, whether the request is for yourself, a loved one, or someone else, and anything you choose to write in the optional message field. When you call or text us, we receive your phone number and whatever you tell us.
Please do not put detailed medical information in the website form. A phone call is the right place for that conversation, and it is protected by the stricter rules described above.
Information collected automatically
Like every website, our hosting provider keeps standard server logs: the IP address of your device, the pages you visit, the time of the visit, and your browser type. When you submit the form we also record the date and time, your IP address, and your browser type alongside the submission, which helps us tell real requests from automated spam.
How we use it
We use form submissions, calls and texts to respond to you: to call you back, answer your questions, check insurance benefits if you ask us to, and help you decide whether our program is a fit. We use the technical information to keep the website running, to block spam and abuse, and to fix problems.
We do not use your information to build advertising profiles, and we do not sell or rent it to anyone.
Who we share it with
Your information reaches only the people and services needed to respond to you:
- Our admissions staff, who receive each form submission by email so they can call you back.
- Our website host, which stores the website and its logs on our behalf.
- Our email delivery provider, which transmits the form notification from the website to our staff.
- Google, whose Maps embed appears on our contact page and whose web fonts the site uses. Loading the map or fonts sends your IP address to Google under Google’s privacy policy. We do not send Google your form details.
Beyond that, we disclose information only when the law requires it, to protect someone’s safety, or with your permission.
How long we keep it
Form submissions are kept on the website server for no longer than twelve months and then deleted. Email copies held by our admissions staff are handled under our internal record-keeping policies and, if you become a client, under the rules that apply to treatment records. Server logs are kept by our host for a limited period and then discarded.
Cookies and tracking
This website sets no cookies of its own and uses no analytics or advertising trackers. The embedded Google Map on the contact page may set cookies controlled by Google. If we add analytics in the future we will update this policy first.
Calls and texts
If you text 813.731.9249, we will reply by text about your inquiry. We do not send marketing text messages, and we will not add you to a texting list. Standard message and data rates from your carrier may apply. Reply STOP at any time and we will stop texting you.
Security
The website is served over HTTPS, so what you type in the form is encrypted between your device and our server. Form submissions are stored outside the public part of the website and are readable only by our staff. No method of transmission or storage is perfectly secure, which is another reason to keep sensitive details for a phone conversation.
Your choices
You may ask us to delete a form submission or to stop contacting you at any time by calling 813.731.9249. If you have become a client, requests about your treatment records are handled as described in our Notice of Privacy Practices.
Children
Our program serves adult men. This website is not directed at anyone under 18, and we do not knowingly collect information from minors through it. A parent or guardian may contact us on behalf of a family member.
Changes to this policy
When we change this policy we will post the new version here and update the date at the top of the page.
